Why are there 3 tasks running each time the trigger runs?
First one it's confirmed.
Next time denied for the same 'task'

As we can see from your task history Id, the email address which you are passing is already added to the respected list of your Aweber account.
Though to avoid that message, you can first search its presence in your Aweber account and then with the help of the Filter condition you can avoid adding it again.
Kindly review your workflow once and give it a try again.
