Hey @Satheesh Arichery

As we can see from your workflow, you haven't configured the trigger event of Google Forms and because of that, your workflow is not getting the new row data.

So, kindly recapture your webhook trigger response again by doing the initial setup correctly and then try again. Let us know if you get stuck at any step.
