I am collecting 2 data from google form, I want to get all data from my clients in one folder with the folder name being the client name

I am creating a google form, where I am asking for the Employee name, client pan card, and client Aadhar card, but in G drive 3 folders are created with form response name, I want to get the folder with Employee name. and inside the Employee name folder, I want to have both pan card and Aadhar card of my Employees
