How can I send data from Facebook leads to my server?

I've a calling server which I want to connect with Facebook Leads. I've a public API available of my server, can I connect it with Facebook leads?
So that as soon as someone fills a lead in Facebook, it's captured by my server

As soon as I click on Endpoint URL, this is what I'm shown. Not sure how to proceed
