How to Generate Certificates with candidate photo using google forms and slides

I have currently generated the certificate using Google Forms and Google Slide on new responses and sent them by mail. Now I need to attach the candidate's photo. I add an upload image option in the form but it only returns the URL of the image that I uploaded. Kindly help me with this.
