Issue with facebook lead ads

Hey team, facing an issue with Facebook lead ads from quite some time.
Sometimes it is sending campaign name and adset name in response but sometimes it doesn't and it becomes very hard to map things as with black response it starts throwing error.
Also the workflow is getting turned off automatically for some weird reason.
What could be the issue?
Is anyone else facing it??
