I am unable to insert data in Test Form second time , So how can i insert lead data in my CRM

Can you please elaborate on your concern briefly, also as we can see you haven't switched on the workflow?

So, please switch it once and then try again.

And since the trigger event's polling time to capture new lead is 8 hours then it will fetch the new lead afterward.
