Sub account set up in GHL

Hey,
Ive gone through the full workflow setup to automate the sub-accounts to be created in GHL. The video I follows was this

.Everything was easy to follow and set up. The only difference I can see between the video and my workflow within Pabbly is there are only two columns (image attached), instead of showing on the labels like shown in the youtube video.
I don't get any errors, but no sub-accounts location or users are being created when we fill in our form.
Any help would be appreciated.
