It appears that previously you have mapped the Trigger Column in place of the Date of Birth field and as a result, it was passing a trigger column from your Google Sheets instead of a Date.
Though it is corrected and working fine. Please give it a try and let us know if the issue still persists.

If you have made any changes in your Google Sheets in which you are getting the Forms' response, kindly try recapturing the webhook response once and then try again.
