Create oppturnity in Flowlu

Hi.
I already created few other workflow that create oppturnity.
But somehow for my new funnel, Submission Cases - CRM - Email is fail to create oppturnity

Although i already all the step from my previous workflow. i still fail to create oppturnity

Can you help?
