I would like the data to be sent according to this scenario
, but instead of presetting, a sheet was used. I need to use a google sheet because I use the formula image in sheets to generate qr code via google https://chart.googleapis.com .

As I mentioned earlier in my comment that whenever you are submitting data through Google Forms then you will get all the responses in Google Sheets automatically. Kindly check the video you will get the same information there as well.
