Salesforce Leads Custom Fields

I have my web form connected via a web hook and I have Salesforce connected to create a lead, but the salesforce lead information is not pulling in the custom fields for the lead.

Is there a way to get the custom fields on the Salesforce lead form to show up so we can populate the data with our web form?
