I am familiar with the basic process of connecting to Facebook, but I am having trouble getting a lead response from one of the Facebook pages, which is the Casatrance Facebook page.

Seems like you might have done some chanegs in the Lead Ad Form and because of that it is not showing any form, so we recommend you to make a new connection and then try again.
