Why are duplicate and incorrect entries appearing in my Google Sheets across 3 different sheets?

We had used details Ujwala (Phone Number: 8147133839), for testing the Google Form. The test entry was entered correctly, but now we are seeing a glitch with this particular entry while processing through Pabbly.

This issue is happening across 3 different Google Sheets: the PV Seller Leads sheet, the PV Buyer Requirement sheet and the GC Meta Leads sheet, so it does not appear to be limited to a single sheet.

Please investigate the Google Sheets integration/mapping and check what is causing this issue.I have attached a screenshot for reference.
